Internists, family practitioners, geriatricians, cardiologists, nurses, social workers, other health care professionals and administrators who care for or manage older patients who may have terminal illnesses, chronic or progressive with a short life expectancy - a year or less.
Upon completion of this activity, participants should be able to:
- Identify limitations of Advance Directives.
- Present two-stage approach to Advance Care Planning.
- Describe components of Physician Orders for Life- Sustaining (POLST) Paradigm.
- Provide participants with tools and information to facilitate implementation of the POLST Paradigm.
