Target Audience:

Internists, family practitioners, geriatricians, cardiologists, nurses, social workers, other health care professionals and administrators who care for or manage older patients who may have terminal illnesses, chronic or progressive with a short life expectancy - a year or less.

Educational Objectives:

Upon completion of this activity, participants should be able to:

  • Identify limitations of Advance Directives (AD)
  • Present two-stage approach to Advance Care Planning (ACP)
  • Describe components of the Pennsylvania Orders for Life-Sustaining Treatment (POLST) Paradigm
  • Provide participants with tools and information to facilitate implementation of POLST clinical process
